The immune system during sepsis

Septic shock is the most severe outcome associated with pathogen infection in the bloodstream. It is a life-threatening condition invariably leading to multiple organ dysfunctions. Currently, septic shock is one of the most frequent causes of death in intensive care units worldwide.

However, it is already known that sepsis-induced multiple organ dysfunction is not a direct effect of the pathogen invasion itself but rather an overreaction of the host immune system against the infection. Many strategies aiming at holding back the extreme response of the immune system have been developed but little progress has been made.

A group of researchers from Santa Catarina, Rio Grande do Sul and So Paulo, in Brazil, have shown that blocking the receptor of bombesin/gastrin-releasing peptide (GRP), a peptide involved with the activation of neutrophil and macrophage immune cells, improves survival in sepsis animal models. Working with RC-3095, an antagonist of the GRP receptor developed by Nobel Laureate Dr Andrew Schally, the group showed that this molecule attenuates the release of the host's exacerbating immune response elements, and reveals a new inflammatory pathway and potential target for new drugs. The study was part of a research effort to investigate additional functions and potential clinical applications for the GRP receptor, initiated by Drs Gilberto Schwartsmann and Rafael Roesler at the Federal University of Rio Grande do Sul.

Led by Drs Felipe Dal-Pizzol and Fabricia Petronilho at the Universidade do Extremo Sul Catarinense, in Brazil, the group has recently teamed up with Drs Andrew Schally and Norman L. Block at the University of Miami Miller School of Medicine to investigate the potential link between GRP and TLR-4, a receptor found in neutrophils and macrophages. TLR-4 is one of the molecules responsible for spreading the word that the body has been invaded by microbes and that something must be done quickly.

In a paper entitled "Gastrin-releasing peptide receptor antagonism induces protection from lethal sepsis: involvement of toll-like receptor 4 signaling" and published in Molecular Medicine, the group shows that RC-3095 reduces the levels of circulating TLR-4 and other immune elements associated with an extreme inflammatory response. Additionally, the group has found that patients with septic shock have greater amounts of GRP if compared to those with less severe forms of sepsis. These higher levels of GRP may explain why septic shock patients develop multiple organ dysfunctions and present the highest mortality rate among sepsis patients.

The study also shows that in animal models, administration of RC-3095 limits the spread of infection beyond the abdominal cavity, indicating the potential of RC-3095 in preventing the complete breakdown of the host's system.

Besides its clinical relevance, the study provides new insights on the roles of key players involved in the complex communication network triggering extreme inflammatory responses. Together, these findings may open new avenues for the development of effective therapeutic strategies to treat sepsis and related inflammatory diseases.

Built in 2009 by Lake Bluff-based Orren Pickell Builders & Designers, the Tobacks? home incorporates basic principles of universal design, including simplicity and flexibility.

By definition, the concept involves making a product or environment ?usable by all people, to the greatest extent possible, without the need for adaptation or specialized design,? according to the Center for Universal Design in Raleigh, N.C.

Universal design at the Tobacks? house begins with stepless entries into the house and garage. The home also has an elevator to the finished basement, a basement bedroom suite for a future caregiver and grab bars in the hallways.

The Tobacks insisted that the house not look institutional. The result is a home that blends function and style. The open floor plan, with 4-foot-wide hallways for easy mobility, is attractive and suitable for all ages and abilities. An accessible bathroom features a walk-in tub and raised toilet and sink. The layout of the kitchen keeps drawers within easy reach, while the center island is at a convenient height for seating.

?Universal design is all about making the house comfortable and safe for everyone in the family,? said David Roberts, president of Roberts Architects & Construction Group Inc. in Evanston. ?It combines home design with (Americans with Disabilities Act) standards. You have the wider doorways, for example, not just for a wheelchair turning radius but for the guy who breaks his leg in a skiing accident.?

Approaching universal design with a client can raise eyebrows, Roberts said. ?I can?t say, ?Boy, you?re getting old, I think you need grab bars,? ? he said. ?But I do ask their needs, and most families have them. You may not have an older or disabled person in the family, but you have a toddler or a visiting grandparent.?

Roberts talked about a client who wanted the freedom to live in her home even if she became sick or injured.

?After her husband died, she wanted to stay in her home with her dog, though friends urged her to go to a retirement home,? he said. ?She could still get upstairs but asked us to build a first-floor bedroom-bathroom, with easy access to the backyard. Since then, she broke her hip but was able to stay.?

Incorporating universal design elements can be as simple as installing illuminated light switches or lever handles instead of door knobs.

?Usually, the homeowner?s reaction is, ?Why didn?t I think of that??? said Art Wehnert of Country Remodelers in Campton Hills, Ill.

A first-floor master suite is the typical starting point for making a house more user-friendly. ?In new construction, it just doesn?t make sense to not have (a bedroom) on the first floor,? Wehnert said. ?For a remodel, we try to save the homeowner money by converting a large unused room like a den. But more often, we need to add a bathroom to it.?

Cheryl Daugvila of Cheryl D. & Co., a kitchen and bathroom design firm in La Grange, said it?s ideal to build a room that can be a den or guest room now, with a Murphy bed and built-in cabinets. Later, the space can become a bedroom with a bed and built-in drawers.

In addition to baby boomers needing first-floor bedroom suites for themselves, more homeowners prepare them for elderly parents, Daugvila said.

?More often, we have more than one generation in the house, so someone in the family needs that room,? she said.

Bathrooms that can accommodate a wide variety of users are safer for everyone, said Chicago-based interior designer Leslie Markman-Stern.

?Forget the polished marble floor of the ?90s; everyone slipped on that,? she said.

For safety, her bathroom designs have textured flooring or smaller tiles because it is the grout, she said, that reduces slipping. She also suggests lights in showers and tubs, towel bars anchored in wall studs, hand-held shower heads and night lights disguised as wall lights.

First-floor master suites are a selling point no matter buyers? demographics, Markman-Stern said.

?They work for nannies, live-in grandparents and parents who don?t necessarily need to be on the same floor as their babies because of the new baby monitors,? she said.

Modifying the kitchen to make it safe and comfortable can mean less bending, reaching and lifting.

?Even without a disability, it helps to have everything at counter height, at that sweet space where you don?t have to climb a stool or bend over,? Roberts said.

Wehnert added, ?All the little things should be thought out, like task lighting where you prepare food and dog bowls in pull-out drawers so you don?t trip over them.?

Designers can help you find products not seen in showrooms, like faucets with touch controls, upper cabinets that pull down, shelves that pull out and appliances with easy-to-see controls.

Consider a counter-height, not bar-height, breakfast counter, Markman-Stern said. ?Then you can use chairs instead of stools that tip over,? she said.

The market for aging-in-place remodeling continues to grow as more homeowners choose to remain in their homes as they age. The National Association of the Remodeling Industry and the National Association of Home Builders offer programs and courses that promote standards of universal design in residential projects.

According to a June study by the American Association of Architects, the percentage of clients who asked their architects for ?flexibility in accessibility? rose to 64 percent in 2012 from 58 percent last year.

D.R. Horton Inc. is one of the production builders that has jumped on the universal-design bandwagon.

Sources: Olympic security checks not properly carried out

LONDON -- Security checks of vehicles entering Olympic sites in the U.K. have not been properly carried out, according to a number of staff, ITV News reported Wednesday.

The unnamed staff claimed that in some cases fake searches of vehicles for bombs were carried out using dogs that had not been trained to detect explosives.?

The company involved, G4S, denied the claims and ITV News reported that some of the staff who spoke to them had been sacked.


The U.K. news station said the ?key claims? were that:

  • Cargo areas of trucks and vans have not been adequately searched by either dogs, scanners or other security teams.
  • In some instances, dogs that were not trained to find explosives were used to carry out fake searches of vehicles.
  • Some assessments of dogs and handlers have been faked.

Ian Horseman Sewell, G4S?s managing director of global events, told ITV News the allegations were not true.

"At no point is there any evidence that dogs that have been trained to detect substances other than explosives have been used to try to detect explosives,? he said.

A 10-Part Strategy for Boosting Daily Productivity | Business 2 ...

Do you begin your day full of grand intentions of all that you want to accomplish, yet somehow end up going to bed feeling as if you?ve gotten nothing done?

Like many of you, my daily priorities vary. But I have some standard mental tricks for staying focused and productive. Following my Morning Routine and Daily Planning, I use these techniques to implement the plan.

After planning and processing, I can?t wait to get things done! Here?s how I make it happen:

  • Follow orders. Each day, I make a number order for my daily activities and try to follow it as closely as possible, so the most important tasks get done early in the day and early in the week. Less pressing activities can be moved to another day, if my schedule changes.
  • Allow for reflection. Stepping away from my computer and thinking about how to solve a problem or write a blog post or reply to a complex email is not procrastination. This reflection time is immensely productive and seems to happen best for me over a snack or a quick walk.
  • Reward myself. To keep up my motivation and to allow for a quicker mental shift between different types of activities, I reward myself with these types of breaks: replying to a few personal emails, a snack or a stretch break.
  • Cut up large projects. At the start of the 2?3 hour blocks of time I set aside for large projects, I scribble down a mini-plan of all of the action steps. Then I implement on that mini-plan within the time block.
  • Evaluate the cost. If a project starts taking longer than expected, I evaluate the cost of finishing it versus moving on to something else or getting it to ?good enough? with the understanding I can come back, if there?s time.
  • Stuff in little activities. I try to do little activities like sending in checks or setting up appointments within small pockets of time between scheduled activities.
  • Watch my people levels. If I?m craving people time, I try to arrange a call or work in an environment where I?m around others. If I feel tired of being around others, then I seek out solitude.
  • Capture the next step. If I leave a voicemail, I make a note on my calendar to follow up. If I leave a meeting, I make a note of when to complete the minutes. If I receive a blog post assignment, I make a note of its deadline and when to start it. Everything has a documented next step.
  • Create urgency. I always think of a few more things that it would be nice to fit in a day or the week than I think I could actually get done. That way, I?m motivated to push myself and see how much I can do, instead of subconsciously delaying because I?m not sure what would be next.
  • Accountability and transparency. I have four individuals that help keep me motivated by holding me accountable. I commit to being transparent with them about what I?m actually getting done, especially in terms of business and professional development. (They send me lots of encouraging affirmation in return!)

Romney: Energy holds key to manufacturing rebirth

President Barack Obama has put obstacles in the path of the natural gas and oil industry and is looking to end the use of coal for energy, says Mitt Romney, the presumptive Republican presidential nominee.

?My view is entirely different than his,? Romney said. ?I think if we aggressively pursue our natural gas and coal and oil resources, they will be in abundance and relatively cheap. You?ll see manufacturing return to America from offshore.?

Romney recently answered questions about energy during a one-on-one interview with a Vindicator reporter in Brunswick, a community in Medina County.

With the oil and gas industry boom in the state, including the Mahoning Valley, and the importance of coal to Ohio?s economy, energy is one of the key issues in this year?s presidential election.

Obama issued an executive order in April to create a federal interagency group to develop a plan for safe and responsible gas development, including fracking.

Fracking is a process in which water, chemicals and sand are blasted into rocks thousands of feet below the ground to unlock natural gas and oil.

?My concern has been there?s been nine different federal agencies all trying to insert themselves into the fracking regulation and it has convinced some employers that natural gas in abundant supply will not be reliable in this country,? Romney said.

When asked about Romney?s statement on the number of federal agencies involved in gas development, his campaign pointed to Obama?s April executive order that calls for high-level officials from at least 13 federal agencies to be a part of the interagency group.

As for concerns from employers about natural gas, Romney said a high-ranking official with Dow Chemicals Co. told him the company decided not to build in Oklahoma and instead go ?offshore because they were worried about the reliable supply of natural gas given the current administration?s intrusion into fracking regulation.?

As for concerns about supply, the Western Reserve Transit Authority is considering adding natural-gas vehicles to its fleet, said Matthew Kotanchek, its director of maintenance.

?There has been nothing political that has caused concerns about supply,? he said.

Federal laws are needed for safety oversight for coal mining and fracking, Romney said.

?There?s a very legitimate and important role that [the federal] government plays in setting the table for the economy to work,? he said. ?Traditionally, fracking regulation has been managed at the state level and has worked well.?

In response, Jessica Kershaw, an Obama campaign spokeswoman, said: ?Mitt Romney?s rhetoric on energy is dishonest and misleading.?

With Obama as president, domestic oil and natural-gas production have gone up every year with oil production reaching an eight-year high and natural-gas production reaching an all-time high in 2011, Kershaw said.

?President Obama believes natural gas is an important part of our all-of-the-above energy strategy, and he?s committed to developing its production in a safe and responsible way,? she said.

With Obama as president, drilling on federal lands has decreased, Romney said.

?Licenses have been cut in half; permits have been cut by two-thirds on federal lands,? Romney said. ?The president said, ?Gosh, look there?s more oil being produced.? Yes, Mr. President, on private land, not as a result of anything you?ve done, but in spite of it.?

When Obama could help, ?he has not encouraged the development of natural gas, oil and coal,? Romney said.

Those involved in the oil and gas industry do not believe the government is trying to stop development, just that the process is slow.

There is little in the way of regulation to prevent companies from drilling for oil and gas on federal land, said Terry Fleming, executive director of the Ohio Petroleum Council.

Recently there have been meetings between companies and federal officials about drilling permits in the Wayne National Forest in southeast Ohio, he said.

?The park officials seemed very interested in drilling,? Fleming said.

When asked about fewer drilling permits being issued on federal land, he said, ?I expect everything to take longer when you?re dealing with the federal government.?

The amount of time it takes to receive a permit is the biggest issue with drilling on federal land, said Jerry James, president of the Ohio Oil and Gas Association.

The regulations have not changed significantly, but the amount of time it takes to get a drilling permit on federal land has increased dramatically during the Obama administration, James said.

?I?m working on some leases on federal land, and we?re a year and a half into the process. We?re being told it may take another year for the permit,? he said. ?Two and a half years is a lifetime in the oil and gas industry. Companies might just move on to other projects.?

It does not appear as though the federal government is against drilling on federal land. The process is simply time-consuming, James said.

Under Obama, coal regulations have ?become more and more labyrinthian, more difficult for coal to be mined and almost impossible to be used,? Romney said.

In January 2008, Obama said, ?If someone wants to build a coal plant, they can. It?s just that it will bankrupt them because they are going to be charged a huge sum for all that greenhouse gas that?s being emitted.?

Romney said the president is ?obviously acting in concert with that view.? He added that ?the president?s priority is to reduce greenhouse gases and that means doing everything you can to shut off? gas, oil and coal.

?My priority is to create good jobs for the American people and keep our air clean,? Romney said.

Romney said with him as president the country will see ?a manufacturing resurgence.?

Romney ?claims he supports coal,? Kershaw said, but the former Massachusetts governor ?has been a longtime critic of coal-fired power plants.?

Just after Romney became governor, he said a coal-fired plant in Salem Harbor, Mass., needed to be closed because ?that plant kills people.? IHSAA Creates 6A Football Class, Sets Format For Moving Schools In Classes

INDIANAPOLIS (Indiana's NewsCenter) - The Indiana High School Athletic Association has created a 6A class for football and established criteria for moving schools up in class in all sports.

The IHSAA Board voted 13-5 to create Class 6A in football effective with the 2013 season, taking the top 32 schools in enrollment statewide out of Class 5A to create it.

The schools moving to 6A locally according to the latest enrollment figures would be Northrop, Homestead, Warsaw, and Snider.

Remaining in 5A as part of the next 32 in enrollment are Carroll, Huntington North, South Side, North Side, and Wayne.

5A and 6A will in the new system have a bye week at the end of the regular season and begin sectional playoff action with semifinals, while the other four classes continue to play quarterfinal games.

In addtion, the IHSAA voted 16-2 to create a two year period for all sports in which teams would accumulate points that could move them up a class with success in competition.

Teams would get one point for a sectional title, two for a regional championship, three for winning a semi-state, and four for being a state champ.

If a team accumulates six points in this system over a two year period, it would be moved up a class for the next two years.

Egypt braces for Islamist president, or army rule

CAIRO (Reuters) - Five hundred days after they overthrew Hosni Mubarak, Egyptians will finally have a new president on Sunday, the first they have chosen freely and who may well be from the Muslim Brotherhood, which Mubarak and fellow generals spent a lifetime fighting.

The result of last weekend's run-off, due in an election committee news conference at 3 p.m. (1300 GMT), will be historic for Egypt and the Middle East. Many think Islamist Mohamed Morsy will become head of state of the biggest Arab nation, reshaping the region after decades of Western-backed military rule, even if the armed forces are not giving up their control just yet.

The Brotherhood may react angrily if it is instead Ahmed Shafik, a former air force commander and Mubarak ally. His victory many Egyptians, and millions across the region, would fear as a mortal blow to last year's Arab Spring revolution, despite his assurances of also wanting an inclusive government.

After an anxious week of street protests at Cairo's Tahrir Square and angry accusations between rivals of subverting the new democracy, the new president will emerge with fewer powers than the candidates, pruned by a first round of voting in May, had expected when the army promised civilian rule from July 1.

The ruling military council, which pushed Mubarak aside to appease the protesters in the streets, has just stripped the post of many powers and dissolved the Brotherhood-led parliament elected in January. Yet the presidency is still a prize, even if the vote will not end the power struggles over Egypt's future.

An Islamist president of Egypt would be a major milestone for the Middle East, near unthinkable 18 months ago. It is far from confirmed. But the military, Brotherhood and other officials gave signs of expecting it will happen.

Brotherhood supporters camped out in Tahrir Square, where the revolution was won, were generally in festive mood, though fear of disappointment still nagged, after decades of rigged elections. In counterpoint, a few thousand rallied on Saturday in a middle-class Cairo suburb to declare support for the army.

Morsy, a 60-year-old, U.S.-educated engineer and political prisoner under Mubarak, declared victory within hours of polls closing last Sunday - a move condemned by the generals. In a sign of continued confidence, he has already met other groups and drafted an accord to form a national coalition government.

His party issued a statement on Saturday saying it had called on "all partners in the nation, from all movements, to take part in this national platform, to guarantee the success of what we have achieved and their active participation in rebuilding the country in the manner it deserves".

One of those involved, Abdel Gelil Mostafa of the reformist National Association for Change, told Reuters on Saturday: "We agreed on a general program, especially for if Morsy won.

"That seems probable. But, we will know tomorrow."

BROTHERHOOD, ARMY TALKS

By contrast supporters of Shafik, 70, who was Mubarak's last prime minister in his final desperate days, kept a low profile, although he did declare publicly on Thursday he was confident.

A victory for Shafik, who won backing in the run-off from many who decided they liked religious rule even less than a candidate drawn from the familiar military establishment, could spark protests from well-organized Islamist movements, which the army and security forces might confront on the streets.

Reformist politician Mohamed ElBaradei said he had been in contact with the army and Morsy's camp to avoid a showdown, but said he was worried that, if Shafik were declared winner, "we are in for a lot of instability and violence ... a major uprising." His comments were carried by CNN on Saturday.

However, there have been indications from senior figures in the Brotherhood and military council that they have prepared for a Morsy presidency in meetings since the election.

While officials deny any negotiation over the long drawn out process of tallying the election results themselves, there would be greater scope for compromise to defuse tension over what many have called the army's "soft coup", against parliament and the powers of the president, if Morsy is made head of state.

The Brotherhood has said it will go on protesting until the military council cancels the dissolution and a decree which gave its legislative powers to itself, the Supreme Council of the Armed Forces (SCAF). But were its candidate to be confirmed in office, then the wary symbiosis between the two old enemies, seen since Mubarak was ousted, may continue in a new form.

It is a collaboration that irks many of the secular liberals who led the first wave of the uprising against Mubarak but found themselves fragmented and eliminated in last month's first round of voting, left with a difficult choice between army and Islam.

MILITARY ROLE

Thousands of Brotherhood supporters were in Tahrir Square again on Saturday, chanting "Victory for Morsy!" and "Morsy, Morsy, Allahu akbar!" (God is greatest), while waving national flags.

"We want the military council to announce the real results without forgery," said Hassan Eissa, 43, an accountant from north of Cairo who was demonstrating on the square.

"They have no right," Eissa said. "Egyptians shouldn't be under any kind of guardianship after the revolution."

The generals, who oversaw Mubarak's departure on February 11, 2011, have repeatedly said, both to Egyptians and to their close U.S. ally, that they will return to barracks and hand over to civilian rule. But they present themselves as guardians of Egypt's security and long-term interests and moved to block the Islamists from taking more than a share of power.

While they have said that inaugurating the new president by July 1 will meet the deadline they set themselves for civilian rule, their moves in the past 10 days to curb the presidency and hang on to a veto over legislation, as well as to claim a role in drafting a new constitution, mean that the process goes on.

Washington and the European Union, both major aid donors, have expressed concern that the military is backtracking. But both also share its anxiety over a sweeping Islamist takeover, worried about Egypt turning anti-Western and reneging on its peace with Israel, and also voicing concern for civil rights.

Violence by hardline Islamists in Tunisia, whose revolt inspired that in Egypt, has troubled many Egyptian liberals.

Key to the transition to democracy will be drafting a constitution. That work has been hostage to partisan wrangling in the now defunct parliament, where one assembly collapsed after complaints it was too Islamist and a second faces a challenge in court on Tuesday that many expect to trigger a clause in the SCAF decree letting it name a new drafting panel.

The Brotherhood has portrayed itself as a modern movement, ready to work with others and willing to respect treaties. Some supporters cite the example of Turkey as a model - a Muslim democracy with a history of military interference in politics, where devout, elected politicians have slowly asserted control.

The bearded, bespectacled Morsy was not a familiar figure to Egyptians. Some ridicule him as the movement's "spare tire"; his campaign began after a more senior leader was barred from the race. Critics say he would be a mere front-man as president, for less visible pillars of the Brotherhood behind the scenes.
